Please read and confirm your understanding of the waiver below:

1. I understand that Jiu Jitsu, Grappling, Self-Defense, and similar activities are physically demanding and carry inherent risks, including the possibility of serious injury or death. These risks can come from my own actions, the actions of others, or the condition of the facilities and equipment.

2. I understand that not every risk can be anticipated. I am choosing to participate voluntarily and accept full responsibility for all known and unknown risks that come with these activities.

3. I confirm that I am in good health and physically capable of participating. I do not have any medical condition that would prevent me from doing so safely.

4. I understand that I am personally responsible for any medical costs resulting from an injury I may suffer, and that I carry or am responsible for obtaining adequate medical insurance.

5. I agree to release and hold harmless Your Journey Jiu Jitsu, LLC, David Miller, its owners, employees, instructors, contractors, and the owners of any premises where training takes place (collectively, the "Released Parties") from any and all claims arising from my participation, including claims of ordinary negligence, to the fullest extent permitted by law.

6. I agree to indemnify the Released Parties against any claims brought by me, my family members, or any third party in connection with my participation.

7. I understand that if I am under 18 years of age, a parent or legal guardian must also sign this waiver on my behalf.

8. I understand that this agreement is governed by the laws of the State of Arizona, and that if any part of it is found unenforceable, the rest remains in full effect.

9. By drawing my name below, I am electronically signing this agreement. I understand that my electronic signature is legally binding, just like a handwritten signature.

Your Journey Jiu Jitsu, LLC may take photos or videos during training, events, and classes for use in promotional materials, social media, and marketing.
